Once we have received your application, a volunteer will be in touch with you to conduct a phone interview. **Please note: we are an all-volunteer organization, and sometimes it will take a few days before you hear from someone. We do our best to contact potential adopters within 72 hours, but it is not always possible. Please be patient with us. Given our strained time resources, we ask that if you are interested in a particular dog that you submit an application first, before writing to inquire about a dog. If you are not inquiring about a specific dog, please enter "none". Are you married, single , have roommates? PET INFORMATION: What is the reason for wanting to adopt a rescue GWP? Have you applied to any other rescue groups, shelters or breeders in the last 6 months? If so, please list. Have you ever personally owned a dog before? Have you ever owned a GWP? Do you still own a GWP? If not, how did you lose your former pets that you have owned within the past 5 years? What other breeds have you owned? Are you looking for a specific gender, male or female? If so, why? Please describe what you are looking for in your new GWP? Are you looking for a primarily pet or hunting companion? What activities do you plan ( if any) participating with your new GWP ( obedience, hunt tests, rally, agility). How do you plan to meet your new GWP’s exercise requirement? Do you have a desired age range? What specific traits are you looking for in your new GWP? Are you willing to attend obedience classes with your new GWP? If not, please explain why not. Do you intend to crate train your new GWP? If not, please explain why. Please list current animals in your household. Include all dogs, cats, birds, rodents, etc. Please include sex, age, etc. If there are no animals currently in the household, please enter "none". How far are you willing to travel to get your new dog? What is your timeline for searching for a dog? (weeks, a few months, up to 6 months, up to a year, no limit) Are your current pets spayed/neutered? If not, please explain why. Have you ever given a pet away, taken a pet to a shelter or returned one to a breeder? If so, please explain the circumstances. How much time will the dog spend alone during the day? Where will your new GWP spend that time? Where will the dog sleep? HOUSEHOLD INFORMATION Who will be the dog's primary caretaker? How many adults are in the household and what are their ages? Are all adults in the household supportive of adopting a GWP? Are there any children in the household and what are their ages? If there are no children, please enter "none" Does anyone living in the home suffer from allergies or asthma? Do you RENT or OWN your home, townhouse, condo, apartment, etc.? If you rent, can you provide written permission from your landlord that you can have a large sized dog housed at your dwelling? Please provide landlord's name address and phone number. FENCING: Do you have a fenced yard? Describe the type of fencing you have (stockade, chainlink, invisible,etc). If traditional fencing, how high? What is the size of the fenced in area? If you do not have any fencing in place, would you be willing to install traditional or invisible fencing? VETERINARIAN INFORMATION: Do you have a regular veterinarian?
