provide foster care for dogs
to provide a safe haven until they are adopted.

All dogs need socialization,
playtime, bath-grooming, medical attention.
All
medication, vaccines and vet care will be provided by Mostly Beagles.
All dogs are spayed/neutered, current on vaccines, wormed, heartworm tested,
given Frontline for flea/tick protection, and are on heartworm prevention prior
to adoption.
Mostly
Beagles will provide the pet’s food if requested.

to help at adoption days, fundraisers, and other special
events.

helping in this manner.
We are looking for volunteers
to help load, unload and transport the dogs to and from the adoption days,
fundraisers and special events. Set-up and tear-down of the event site help
is also needed.

I,
the undersigned, individually and/or as a parent/guardian of the below listed
name, do personally and on the behalf of a minor, assume all risks and hazards
incidental to such activities.
I, the below
named Volunteer, understand that Mostly Beagles. and its representatives have
limited information regarding the temperament and habits of the dogs under
its care and ownership. As a volunteer for Mostly Beagles I agree to take
all reasonable precautions (through the use of crates, leashes, supervision,
and common sense) to ensure that the dog does not run loose, become a
nuisance, destroy property, injure or be injured by another dog, be injured
by any person, or injure me or another person.
By becoming a
Volunteer, I hereby waive, release, discharge, and hold harmless Mostly
Beagles and its agents, employees, directors, representatives and other
volunteers charged or chargeable with liability, their heirs, administrators,
executors, successors, and assigns, from any and all claims, damages, costs,
expenses, loss of services, actions, and causes of actions, whether known or
unknown, belonging to me, my agents and assigns, due to any action or
occurrence or damage which arises in connection with my volunteer activities,
including, but not limited to dog bites to me by a Mostly Beagles dog.
Furthermore, I agree to pay Mostly Beagles any and all expenses, including
court costs and reasonable attorney fees, incurred in enforcing the terms and
provisions of this agreement.
The UNDERSIGNED
hereby acknowledges having read this Waiver and does hereby agree to abide to
its terms as set forth above.
