3. Your email address
4. Are you living in a house, condo or apartment?
5. Do you own or rent?
6. Do you understand that cats must be kept inside at all times?
7. Why are you interested in adopting this particular animal or animals? How do you see this animal becoming a part of your life? What things would you like to be doing together?
8. Give the names, species, sex, age size and personality of all other animals who live with you.
9. Have other animals been part of you family in the past? What happened to them?
10. Give names, ages and experience with animals for any children who live in your home. Please specify any special needs or desires your children have.
11. Describe the work and activity schedule of your family.
12. Your veterinarian's name, address & phone
13. A family/home visit is required for all adoptions. All family members must be present during the visit. We use this opportunity to help us get to know you so that we can make a placement that will be compatible leading to permanent home for the animal. Do you agree to the family/home visit?
14. We finalize all adoptions with an Adoption Agreement and a donation is accepted in the animal's name.
